Q. Are the specimens available right away?
A. We usually procure from surgical cases, but we have a
tissue bank that we can serve from once the application
is approved depending on the tissue requested.
Q. What kind of tissue do you provide?
A. Any kind of remnant tissue that can be provided from
the OR.
Q. How is tissue prepared?
A. We have fresh, frozen, fixed and paraffin blocks. We
also can add slides if requested.
Q. Do you test the tissue for HIV or Hepatitis C?
A. No, we don't test for HIV or Hepatitis C, but we don't
procure tissue knowing that it is HIV or Hepatitis C infected.
Q. How long does it take to be approved and start receiving
tissue?
A. It takes 5 business days once we get the complete application.

Q. What is the easiest way to be billed?
A. The easiest way to be billed is by Standing PO.
Q. Are the shipping charges included in the price?
A. No, the investigator handles the shipping charge.
Q. Which courier service do you use for shipping the specimens?
A. We use FedEx Express overnight delivery. We also get requests
from investigators to use UPS.

Q. How do you add / change a request?
A. Traci Jordan is in-charge
for tissue requests and all questions regarding changes
in requests must be directed to her.
Q. I placed my request on hold (on date), why should I
accept the specimens?
A. An investigator is required to accept all specimens procured
before the CHTN was notified about the hold.
Q. How long it will take to get some specimens?
A. It varies from tissue to tissue depending on the availability.
Q. How long will it take to get "breast" (or other) tissues.
A. No specific time frame is set. As soon as a particular
tissue is available, the investigator is notified.
Q. What type of information comes with the specimens?
A. A complete anonymized pathology report.
Q. Can I get additional patient information?
A. No, unless you have informed consent (i.e., clinical trials).
External investigators CANNOT under any circumstances get
this information.
Q. Whom do I contact to add / change a request ?
A. You need to contact Traci Jordan.
Q. How much will shipping charges be?
A. Shipping charges depend on the location, weight of the
package, value of the package, contents of the package.
Q. What requests do I have in my profile?
A. Information about your profile can be obtained from
Traci Jordan.
Q. What additional services do you have?
A. Tissue banks containing frozen and fixed tissues.
Q. My package isn't delivered yet ....where is it?
A. The tracking number for tissues shipped is provided and
questions regarding delivery must be directed to FedEx.
Q. How do I get QC results?
A. Email or fax a list of CHTN ID numbers to Traci Jordan.
However, please note that we cannot get QC results for specimens
shipped from other divisions of CHTN.
Q. How do you store frozen specimens?
A. Specimens are stored in a -80 degree C freezer. Long term
storage is done in vapor-phase liquid Nitrogen tanks.
Q. Can you hold my specimens until you procured more?
A. Maximum of 5 specimens can be kept on hold at a time due
to the high quantity of specimens that are stored in our facility.
Q. Can I get a list of all specimens that have been shipped
to me?
A. Yes, the list of specimens obtained from the Midwestern
division can be obtained. Call (614) 293-5493 or (614) 293-5119.
Q. Can you ship my fresh specimens with ice packs instead
of bags of ice?
A. No, we do not use ice packs.
Q. How will I be notified about the delivery date of my
specimens?
A. You will be notified by our shipping office.
Q. Is the packing slip also the invoice?
A. No, the packing slip is not the invoice.
Q. If I do not have a Fedex account, how will I be charged
for shipping?
A. Shipping charges will be added to the invoice.
